I didn't find out I was pregnant until I was around 8 weeks, and before this I would always talk about babies, and my boyfriend would always say it wasn't the right time. I dated my wife for two years, prior to marriage. Been married for 8 years now. We have 1 beautiful daughter, and bought a house in the city she has always wanted to live in. It’s one thing to leave a downer partner you’re casually or seriously dating in order to take care of your own needs, but what happens when you meet and marry a happy person, start having a wonderful life and children together, and then your previously happy partner starts becoming moody/depressed?  What if the depression is caused by a terminal illness or some other life-altering event?  Do you get a divorce so you can take care of yourself?  
I was reading your blog and your patient's brother sounds so much like mine. It's so frustrating and heartbreaking, especially when there are children involved.
